Root Cause Analysis

Seminar Content

This class teaches a basic understanding of each step in Quality Techniques’ proven problem-solving methodology. Participants will learn the skills necessary to analyze a problem to determine its root cause, to determine the appropriate action to eliminate the cause, to verify the effectiveness of the action taken, and to ensure that improvements are built into the system.

This class is a must for any company that wishes to optimize its ISO 9001-based system to achieve the maximum return on investment. Participants are taught how to avoid putting “band-aids” on the problems identified in their corrective action system, and how to minimize the chance of those problems recurring.

Objectives of this training are to:

  • Learn how to apply a proven problem-solving methodology to address the major opportunities for improvement in your organization;
  • Optimize (and quantify) the successes for a corrective action process developed for ISO 9001, ISO/TS 16949, ISO 14001, OHSAS 18001, or other ISO-based system;
  • Learn how to prevent the recurrence of major quality, environmental, or OH&S nonconformities in your organization.

The scope, length, and specific content of the class are always customized to meet the individual needs of the attendees and the organization. The standard agenda addresses:

  • Introduction
  • Course objectives
  • Application of problem-solving methodologies to management systems
  • Forming an effective team
  • An overview of effective team management skills
  • An overview of the problem-solving methodology
  • Step 1: Clearly Define the Problem
  • Agree on what the problem is and is not
  • Single event vs. process problems
  • Determine current status so that improvement can be measured if possible
  • Step 2: Understand the Process Being Studied
  • Flow charts
  • Process models
  • Step 3: Determine the Root Cause
  • Brainstorm
  • Check sheets
